Schedule Online
Use our online scheduler to instantly schedule your patient. We strongly recommend you and your patients do this first to secure the soonest appointment time. Except in urgent cases, there are no appointments available sooner than the online schedule shows.
Send Referral By Online Form
Ready to send referral information like it’s actually the 21st century? Our online form is now our preferred method for referrals, for any other health professional who would like to communicate regularly and directly with our office regarding patient care.
Send Referral By Fax
Primary care and other health care providers may still send a traditional fax referral to 253-479-0104. Patients do not usually require a referral for eye examinations or vision services, and patients can feel free to schedule online without a referral or before a referral is sent.
Referrals for Low Vision Services
If you are an Optometrist or Ophthalmologist referring a patient for Low Vision Services, please send a report or chart note with the patient’s most recent ocular and retinal health findings.
